At Monroe Plan for Medical Care, we open doors to better health for underserved populations. We serve individuals and families who need health care coverage but may not have access to or be readily able to afford employer sponsored or direct pay health insurance. We pride ourselves on working with these Medicaid and safety net populations by combining the right-touch approach to disease management with state-of-the-art tracking and predictive modeling, enabling us to help more people get better care at a lower cost.
Monroe Plan has over 225,000 members, who are located in and around the Rochester, Buffalo, Binghamton, Syracuse, Utica, and Watertown areas in New York State who come to us through partner organizations whose safety net programs we manage. Once a person enrolls in Medicaid Managed Care, Child Health Plus, or Family Health Plus with one of our partner organizations, Monroe Plan steps in to manage that person's health care. Our employees take an active role in facilitating access to high quality health care services for all our members, empowering members to take an active role in their health, fostering effective partnerships between patients and providers, supporting caregivers in delivering high quality services, and collaborating with community groups on issues affecting the health needs of patients and their families.
